Scheduling a Blood Draw for the Precivity™ Test
Call 1.877.275.2012 to schedule your blood draw appointment. To make an appointment, you will need the following:
Your Precivity™ test kit number (you will need this for the call)
A signed copy of the Requisition Form (signed by patient and healthcare provider) before your blood will be drawn
(Please note that these services may not be available in all locations.)
Patients who have received their Precivity™ test kit from their healthcare providers may schedule a blood draw appointment.
